替吉奥胶囊联合化疗治疗老年晚期胃癌的临床观察

      目的 分析替吉奥胶囊联合化疗治疗老年晚期胃癌患者的效果.方法 选取54例老年晚期胃癌患者,通过随机数字表法分为对照组和观察组,每组27例.对照组采用奥沙利铂+5-氟尿嘧啶+亚叶酸钙化疗方案,观察组在对照组化疗方案基础上加用替吉奥,2组均治疗3个疗程,2周/疗程.对比分析2组的临床疗效、肿瘤标志物[癌胚抗原(CEA)、CA125、CA199]水平变化和不良反应发生情况.结果 治疗后,与对照组对比,观察组治疗有效率(48.15％)和肿瘤控制率升高,但2组差异无统计学意义(P＞0.05);观察组CEA、CA125和CA199水平降低(P＜0.05);观察组白细胞减少率和胃肠道反应率均下降(P＜0.05),观察组血小板减少率、周围神经毒性率和血红蛋白减少率降低,但2组差异无统计学意义(P＞0.05).结论 采用替吉奥胶囊联合化疗方案治疗老年晚期胃癌患者效果显著,可有效降低血清中肿瘤标志物水平,且安全性较高.

      Objective To explore the effect of the combined treatment with S-1 capsule and chemotherapy on elderly advanced gastric cancer.Methods 54 elderly patients with advanced gastric cancer were selected and randomly divided into 2 groups:control group and observation group,27 patients in each;chemotherapy with oxaliplatin + 5-fluorouracil + calcium folinate was applied to patients in both groups while S-1 capsule was added to patients in observation group;patients in both groups went through 3 courses,2 weeks per course;a comparative study was made to the clinical manifestations before and after treatment,the changes in the levels of carcinoembryonic antigen (CEA),CA125 and CA199 and the occurrence of side effects between the 2 groups.Results After treatment,the effective rate in observation group was 48.5％ and tumor control rate went up while the comparison with those in control group was of no statistical difference (P＞0.05);the levels of CEA,CA125 and CA199 in observation group decreased as well as the leukopenia and the gastrointestinal reaction,the difference between the 2 groups was of statistical significance (P＜0.05);thrombocytopenia,peripheral neurotoxicity,hemoglobin reduction of the patients in both groups dropped down while the difference between the 2 groups were of no statistical significance (P＞0.05).Conclusions The combined treatment with S-1 capsule and chemotherapy is of obvious effect on elderly advanced gastric cancer and can effectively reduce the levels of tumor markers with good safety.
