      目的 评估高强度超声聚焦(High-Intensity focused ultrasound,HIFU)在治疗老年胰腺癌患者的疗效和安全性.方法 纳入2013年1月至2016年1月间,在复旦大学附属华东医院就诊的老年胰腺癌患者73例,患者年龄60 ～81岁,平均年龄70.3±5.6岁.均接受HIFU治疗,观察治疗后1周、6周和3个月患者的肿瘤体积变化、疼痛缓解程度和不良反应的发生情况,并随访记录患者的总体生存率(overall survival,OS).结果 中位随访期为10个月,患者疼痛缓解率为81.2％,术后6周至3个月患者平均疼痛评分降低.术后6周到3个月患者的肿瘤体积减小.HIFU术后中位生存时间为10.5个月,3个月和6个月的OS分别为100％和79.5％.在随访期未观察到严重不良的事件.结论 HIFU作为一种无创的治疗方法,能够有效地控制晚期胰腺癌的肿瘤生长,无明显不良反应.是老年胰腺癌患者安全有效的治疗选择.

      Objective To evaluate the curative effect of high-intensity focused ultrasound (HIFU) on elderly patients with pancreatic cancer and the safety.Methods 73 elderly patients with pancreatic cancer hospitalized during period from Jan.,2013 to Jan.,2016 were selected,aged from 60 to 81 with the average age of 70.3±5.6 years;HIFU was performed on all the patients;observation was made to the changes of tumor volume,degree of pain relief and the occurrence of side effects at the time point of 1 week,6 weeks and 3 months after treatment and the overall survival (OS) was calculated through follow-up.Results The average length of follow-up was 10 months;the rate of pain relief was 81.2％;the average pain score within the period 6 weeks to 3 months after treatment decreased obviously and the tumor volume reduced significantly;the average survival time after HIFU was 10.5 months,OS at the time point of 3 months after HIFU was 100％ and that at the time point of 6 months was 79.5％;no occurrence of serious side effects was found.Conclusions As a noninvasive and effective treatment,HIFU can effective slowdown the growth of advanced pancreatic cancer,relieve the pain,prolong the survival time of the patients and lead to no obvious side effect,it is effective and safe for elderly patients with pancreatic cancer.
