      目的 探讨早期动态MRI增强斜率值对老年糖尿病患者肌肉骨骼系统良恶性肿瘤的鉴别诊断价值.方法 选取湖北省恩施州中心医院西医部老年糖尿病肌骨肿瘤患者64例,根据病理诊断结果分为良性肿瘤组和恶性肿瘤组,应用早期动态MRI(磁共振成像)诊断,对比2组患者MRI增强斜率值的区别.结果 2组患者的诊断结果对比方面,良性肿瘤组和恶性肿瘤组的slope存在显著区别(P＜0.05),DER对比无明显区别(P＞0.05);与病理诊断结果比较,不同临界点slope值在鉴别恶性肌骨肿瘤性质方面存在不同的效果(P＜0.05).结论 早期动态MRI增强斜率值能够体现老年糖尿病肌肉骨骼系统肿瘤血管化程度与灌注,从而有效判断肿瘤的性质,可为临床诊治提供参考.

      Objective To explore the value of early dynamic MRI enhanced slope value in differentiating benign musculoskeletal tumors from malignant ones in the elderly with diabetes.Methods 64 elderly patients with diabetic musculoskeletal tumors,according to the pathological study,were divided into 2 groups:benign group and malignant group;early dynamic MRI (magnetic resonance imaging) was performed on cases in both groups and a comparative study between the 2 groups was made to the difference of MRI enhanced slope values.Results There existed an obvious difference of slope values in the diagnosis between the 2 groups (P＞0.05) while no obvious difference in DER was found (P＞0.05);the comparative study of the pathologic diagnosis indicated that the slope value at different critical point was of different effect in differentiating the malignant musculoskeletal tumors (P＜0.05).Conclusions Early dynamic enhanced MRI slope value can depict the tumor tissue vasculation and perfusion in elderly patients with diabetic musculoskeletal tumors so as to make differential diagnosis of the tumor and to provide reference for clinical treatment..
