老年排便异常患者血清5-羟色胺及生长抑素与肛门直肠动力学相关性的研究

      目的 检测排便异常的老年患者血清中5-羟色胺(5-HT)、生长抑素(SS)的表达水平及其与肛门直肠动力学各项指标的相关性.方法 收集自2016年10月至2017年2月就诊符合老年排便异常的患者75例,其中老年便秘患者40例,男性22例,女性18例,平均年龄(76.1±9.4)岁;老年腹泻者35例,男性19例,女性16例,平均年龄(69.7±9.3)岁.采用酶联免疫吸附实验法测定人组患者血清中5-HT、SS的含量,灌注式肛门直肠测压装置测定患者肛管静息压、肛管最大缩榨压、初始感觉阈值、首次便意感阈值、最大耐受容量.应用Pearson相关系数描述两者之间的相关性.结果 老年便秘患者血清中SS的表达水平与其肛管最大缩榨压呈负相关(r缩榨压=-0.402,P＜0.05),其余指标之间均无相关性(P均＞0.05);老年男性便秘患者血清中SS的表达水平与其肛管最大缩榨压呈负相关(r缩榨压=-0.459,P＜0.05),其余指标之间均无相关性(P均＞0.05);对于老年腹泻患者血清中5-HT、SS的表达水平与其肛门直肠动力学指标而言均无相关性(P均＞ 0.05);不同性别老年腹泻患者血清中5-HT、SS的表达水平与其肛门直肠动力学指标均无相关性.结论 老年便秘患者血清中SS的表达水平可能是影响部分肛门直肠动力学指标的因素之一,血清中SS的表达水平对老年男性便秘患者肛门直肠动力指标有一定影响.

      Objective To detect the levels of 5-hydroxytryptamine (5-HT),somatostatin (SS) in the serum of the elderly with abnormal defecation and the data of anorectal motility so as to explore the correlation.Methods 75 elderly patients were selected during the period from Oct.,2016 to Feb.,2017,including 40 patients with constipation (22 male and 18 female with the average age of 76.1 ±9.4) and 35 patients with diarrhea (19 male and 16 female with the average age of 69.7 ±9.3);enzyme-linked immunosorbent assay (ELISA) was applied in detecting the content of 5-HT and SS in the serum of the patients while anorectal manometry was applied in detecting the data of anal pressure,maximal squeeze pressure of anal,volume of initial sensation,volume of first defecation sensation and maximal tolerable volume;the correlation of the serum content of 5-HT and SS with the data of anorectal motility was described by Pearson correlation coeffecient (PCC).Results The level of SS in the serum of the elderly patients with constipation was negatively correlated to the data of maximal squeeze pressure of anal (maximal squeeze pressure r =-0.402,P ＜ 0.05) and there existed no correlation among other data (P ＞ 0.05);the level of SS in the serum of the elderly male patients with constipation was in negative correlation with the data of maximal squeeze pressure of anal (maximal squeeze pressure r =-0.459,P＜0.05) and there existed no correlation among other data (P＞0.05);the levels of 5-HT and SS in the serum of the elderly patients with diarrhea and those of elderly patients with diarrhea in different gender were of no correlation with the data of anorectal motility (P＞0.05).Conclusions The level of SS may be one of the influence factors on the data of anorectal motility in elderly patients with constipation,and it is of certain effect on data of anorectal motility of elderly male patients with constipation.
