电视胸腔镜肺叶切除术治疗老年早期非小细胞肺癌的临床观察

      目的 探讨电视胸腔镜肺叶切除术在老年早期非小细胞肺癌中应用效果.方法 选取69例老年早期非小细胞肺癌患者,根据手术术式分为对照组(n=34)和观察组(n=35),对照组采用常规开胸肺叶切除术治疗,观察组予以电视胸腔镜肺叶切除术治疗,观察并比较2组手术情况(术中失血量、淋巴结清扫个数和手术时长)、术后康复情况(术后胸引管带管、下床活动和住院时间)、并发症发生率及癌胚抗原(CEA)变化情况.结果 2组手术时长和淋巴结清扫个数对比,差异无统计学意义(P＞0.05),观察组术中失血量明显少于对照组(P＜0.05);观察组胸引管带管、下床活动及住院时间均明显少于对照组(均P＜0.01);术后观察组血清CEA水平低于对照组,差异有统计学意义(P＜0.05);观察组并发症发生率(5.71％)明显低于对照组(26.47％)(P＜0.05).结论 对老年早期非小细胞肺癌患者给予电视胸腔镜肺叶切除术治疗,临床疗效较佳,具有一定的应用价值.

      Objective To explore the effect of video-assisted thoracoscopic surgery (VATS) on the elderly with early non-small cell lung cancer (NSCLC).Methods 69 elderly with early non-small cell lung cancer were selected and divided into 2 groups:observation group (n=35,VATS was performed) and control group (n=34,conventional open lobectomy was performed);observation was made to the surgical performance,postoperative recovery,occurrence of complications and the changes of carcinoembryonic antigen (CEA) of the elderly in both groups and a comparative study was made between the 2 groups.Results There existed no difference of statistical significance in operation time length and the numbers of lymph nodes removed between the 2 groups (P＞0.05);the blood loss volume in operation in observation group was much smaller than that in control group (P＜0.05);the of the chest tube indwelling time,off-bed activity time and hospitalization duration in observation group were shorter than those in control group (P＜0.01);the level of postoperative CEA in observation group was lower than that in control group and the difference was of statistical significance (P ＜ 0.05);the occurrence of complications in observation group was 5.71％,much lower than that in control group (26.47 ％) (P ＜ 0.05).Conclusions VATS is of better curative effect on the elderly with early non-small cell lung cance,and it is of certain application value.
