      目的 探讨老年糖尿病肾病中NOD样受体蛋白3(NLRP3)炎性体在外周血中的表达及与肾损伤指标的关系.方法 采用实时PCR技术检测60例老年糖尿病肾病患者和40例老年健康人组外周血中NLRP3炎性体基因,同时检测尿微量白蛋白、转铁蛋白、免疫球蛋白G、α1-微球蛋白和N-乙酰-β-D-氨基葡萄糖苷酶的水平.分析NLRP3炎性体与肾损伤指标之间的相关性.结果 与老年健康组比较,NLRP3、凋亡相关斑点样蛋白(ASC)以及Caspase-1在老年糖尿病肾病组中表达均明显升高(均P＜0.05);与老年健康组比较,早期肾损伤指标尿微量白蛋白(mAlb)、转铁蛋白(TRF)、免疫球蛋白(IgG)、α1-微球蛋白(α1-MG)和N-乙酰基β-D-葡萄糖苷酶(NAG)在老年糖尿病肾病组中表达均明显升高(P＜0.05).60例糖尿病肾病患者的NLRP3与mA1b呈正相关(r=0.880,P＜0.05)、与TRF呈中度正相关(r=0.560,P＜0.05)、与α1-MG呈低度正相关(r=0.332,P＜ 0.05).结论 NLRP3炎性体可能与老年糖尿病患者肾损伤密切相关,为老年糖尿病患者肾损伤的诊断提供新依据.

      Objective To explore the expression of the NOD-like receptor 3 (NLRP3) inflammasome in peripheral blood and its correlation to renal injury indicators in the elderly with diabetic nephropathy.Methods Real-time PCR was ap plied in detecting NLRP3 gene in 60 elderly with diabetic nephropathy (DN group) and 40 healthy elderly (norm group),and the contents of urine microalbumin (mAlb),transferring (TRF),immunoglobulins G (IgG),alpha-1-microglobulin (α1-MG) and N-acetyl-β-D-glucosaminidase (NAGase) were detected as well.Results The expression levels of NLRP3,apoptosis-associated speck-like protein containing CARD (ASC) and Caspase-1 in DN group were much higher than those in norm group (P＜0.05) and the expressions of early renal injury indicators including urine mAlb,TRF,IgG,αtl-MG and NAGase in the elderly in DN group increased much more significantly than those in the elderly in norm group (P＜0.05);NLRP3 of the 60 elderly in DN group was in positive correlation to mAlb (r=0.880,P＜0.05),in moderate positive correlation to TRF (r=0.560,P＜0.05) and in slight positive correlation to α1-MG (r=0.332,P＜0.05).Conclusions NLRP3 inflammasome may be in close correlation with renal injury indicators of the elderly with diabetic nephropathy,which provides a new basis for the diagnosis of renal injury of elderly diabetic nephropathy.
