      目的 分析外口切开盲端旷置内口结扎术在老年复杂性肛瘘患者中的临床应用价值及优势,以寻求有效且实用的治疗方法.方法 选取复杂性肛瘘初次手术老年患者68例,通过随机数字表法分为观察组和对照组,每组34例.观察组采用外口切开盲端旷置内口结扎术,对照组采取传统肛瘘切开引流挂线术.观察并比较2组的创面愈合时间、住院时间、治疗费用、术后疼痛和术后肛门功能等多项指标.结果 观察组在愈合时间、住院时间和缓解术后疼痛等方面均明显优于对照组P＜0.05、P＜0.01,但在对术后肛门功能影响和住院费用方面与对照组的差异无统计学意义(均P＞0.05).结论 外口切开盲端旷置内口结扎术治疗老年性复杂性肛瘘,在缩短愈合时间和住院时间,减轻术后疼痛等方面有一定的优势,具有一定的临床应用价值.

      Objective To analyze the clinical value and advantages of external incision,blind end exclusion and internal ligation in the treatment of the elderly patients with complex anal fistula.Methods 68 elderly patients with complex anal fistula were randomly divided into 2 groups:observation group and control group,34 in each;external incision,blind end exclusion and internal ligation were performed in observation group while traditional anal fistula incision and seton drainage were perform in control group;the wound healing time,hospitalization duration,treatment cost,postoperative pain and postoperative anal function of the patients in the 2 groups were observed and compared.Results The healing time,hospitalization duration and postoperative pain of the patients in observation group were much superior to those of the patients in cont rol group (P＜0.05) while there existed no significant difference in anal function and treatment cost between the2 groups (P ＞0.05).Conclusions The treatment of external incision,blind end exclusion and internal ligation for complex anal fistula of the elderly is of certain advantage in shortening the healing time and hospitalization duration and in reducing postoperative pain,it is worthy of wider clinical application.
