      目的 探讨老年直肠癌患者术后吻合口瘘早期炎症因子的表达.方法 2015年6月-2017年9月,选取180例老年直肠癌患者,检测术后骶前引流液中肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素1β(interleukin-1beta,IL-1β)、白细胞介素6(interleukin-6,IL-6)、C-反应蛋白(C-reactive protein,CRP)、降钙素原(procalcitonin, PCT)、基质金属蛋白酶-8(matrix metalloproteinase-8,MMP-8)及基质金属蛋白酶-9(matrix metalloproteinase-9,MMP-9)的表达.结果 根据患者术后有无发生吻合瘘分为瘘组(n=20)与未瘘组(n=160),2组患者在年龄、性别和体温等方面差异无统计学意义.瘘组患者炎症因子TNF-α、IL-1β、IL-6、CRP、PCT、MMP-8和MMP-9高于未瘘组患者,差异有统计学意义(均P＜0.05).结论 直肠癌术后炎症因子TNF-α、IL-1β、IL-6、CRP、PCT、MMP-8、MMP-9的表达可能提示吻合口瘘的发生率.

      Objective To explore the expressions of inflammatory cytokines of early postoperative anastomotic leakage in elderly patients with rectal cancer.Methods 180 elderly patients with rectal cancer hospitalized during the period from Jane,2015 to Sept.,2017 were selected and detections were made to the postoperative expression of tumor necrosis factor-α (TNF-α),interleukin-1 beta (IL-1β),interleukin-6 (IL-6),C-reactiveprotein (CRP),procalcitonin (PCT),matrix metalloproteinase-8(MMP-8) and matrix metalloproteinase-9(MMP-9).Results Based on the occurrence of anastomotic leakage,the patients were divided into 2 groups:leakage group (n =20) and non-leakage group (n =160);there existed no statistical difference in age,sex and body temperature of the patients between the 2 groups;the expressions of TNF-α,IL-1β,IL-6,CRP,PCT,MMP-8 and MMP-9 in patients of leakage group were higher than those in patients of non-leakage group withstatisticaldifference (P＜0.05).Conclusions The expressionsofTNF-α,IL-1β,IL-6,CRP,PCT,MMP-8 and MMP-9 in patients after resection of rectal cancer may indicate the occurrence of anastomotic leakage.
