      目的 分析老年喉癌全喉切除术患者焦虑抑郁及生活质量的特点,评价手术全程各阶段精细化的综合护理的干预效果,为其防治提供有效方法.方法 收集行全喉切除术的60例喉癌患者的病例资料,分为观察组(n=30)和对照组(n=30).对照组给予手术全程常规护理,观察组给予手术全程各阶段精细化的综合护理.分别使用SAS量表和SDS量表对2组的焦虑和抑郁进行评分,以及使用生活质量评估量表对2组生活质量进行评估,并进行统计学分析.结果 入院时2组的SAS及SDS评分差异无统计学意义(P＞0.05);出院时2组的SAS及SDS评分比入院时均显著降低(P＜0.05),观察组的SAS评分及SDS评分均显著低于对照组(均P＜0.05).观察组的生活质量评分明显高于对照组(P＜0.05).结论 老年喉癌全喉切除术患者采取手术全程各阶段精细化的综合护理,可以不同程度地改善患者的焦虑和抑郁,提高其生活质量,具有一定的临床应用价值.

      Objective To analyze the characteristics of anxiety,depression and quality of life in the elderly patients of total laryngectomy,and to evaluate the intervention effect of comprehensive nursing at each stage during operation.Methods The clinical data of 60 cases of total laryngectomy were collected and the cases were divided into 2 groups:observation group (n=30) and control group (n=30);routine nursing was performed on patients in control group during operation while comprehensive nursing care was performed on patients in observation group at each stage during operation;SAS scale and SDS scale were applied in estimating the anxiety and depression of the patients in both groups while the quality of life scale in evaluating d the quality of life;all the data were statistically analyzed.Results There was no significant difference in SAS and SDS scores at admission between the 2 groups (P＞0.05) while the scores in both groups at discharge were significantly lower than those at admission (P＜0.05);SAS scores and SDS scores in observation group were much lower than those in control group (P＜ 0.05);the scores of quality of life in observation group was obviously higher than those in control group (P＜0.05).Conclusion Comprehensive nursing care for patients of total laryngectomy at each stage during operation can improve anxiety and depression of the patients at different degrees and can promote the quality of life,which is of certain clinical application value.
