      目的 探讨内镜黏膜下剥离术(ESD)治疗老年人早期胃癌和癌前病变的有效性和安全性.方法 回顾性分析2016年1月-2017年6月接受ESD治疗的早期胃癌和癌前病变患者的临床资料,分成老年组(≥65岁)和非老年组(＜65岁),观察比较2组的ESD的治疗情况、临床疗效和安全性.结果 老年组的平均手术时间为(74.5 ±20.0) min,非老年组为(66.5±18.1) min,两组的差异无统计学意义(P＞0.05).老年组ESD并发症的发生率、病变的整块切除率、完全切除率和治愈性切除率分别为2.2％、100％、95.6％和95.6％,与非老年组的2.4％、100％、97.6％和97.6％相比差异均无统计学意义(P＞0.05).结论 ESD治疗老年人早期胃癌和癌前病变是安全有效的.

      Objective To evaluate the efficacy and safety of endoscopic submucosal dissection (ESD) in the treatment of the elderly with early gastric cancer and precancerous lesion.Methods A retrospective analysis was made to the clinical data of the patients with early gastric cancer and precancerous lesion treated by ESD;the patients were divided into 2 groups according to the age:elderly group (≥ 65 years old) and non-elderly group (＜ 65 years old);ESD treatment,clinical effect and the safety in both groups were observed and a comparison was made between the 2 groups.Results The average operative time in elderly group was (74.5±20.0) min while that in non-elderly group was (66.5 ± 18.1) min,the difference was of no statistical significance (P＞ 0.05);the occurrence of ESD complications,the en bloc resection rate,the complete resection rate and the curative resection rate in elderly group were 2.2％,100％,95.6％ and 95.6％ respectively with no statistical difference from those in non-elderly group (2.4％,100％,97.6％ and 97.6％ respectively) (P＞0.05).Conclusions ESD is safe and effective in treating early gastric cancer and precancerous lesions in the elderly.
