      目的 比较老年支气管扩张症急性加重期住院患者治疗前后痰中性粒细胞弹性蛋白酶活性,以评估患者的临床疗效,旨在为疗效评估提供方法.方法 选取2021 年12 月—2022 年10 月在上海市浦东新区浦南医院因支气管扩张症急性加重住院的患者80 例,均予以抗感染、化痰等治疗14 d,比较用药前、用药 14d后血白细胞计数(WBC)、中性粒细胞计数(N)、C反应蛋白(CRP)水平、降钙素原(PCT)水平、痰中性粒细胞弹性蛋白酶(neutrophil elastase,NE)活性等差异,以及痰NE活性与上述指标值的相关性.结果 患者治疗前后上述各项炎症指标值均有差异,治疗后均较治疗前下降(P＜0.05);痰NE活性治疗前后差值与N、CRP、PCT水平治疗前后差值呈正相关性,与WBC差值无相关性.结论 痰NE活性有望作为气道炎症标志物用来评估老年支气管扩张症急性加重期患者的治疗效果,具有一定的临床应用价值.

      Objective To compare the activity of sputum neutrophil elastase(NE)of hospitalized elderly patients with acute exacerbation of bronchiectasis before and after treatment,in order to evaluate the clinical efficacy of patients and provide a method for efficacy evaluation.Methods A total of 80 patients with acute exacerbation of bronchiectasis who were hospital-ized in Punan Hospital in Pudong New District from December 2021 to October 2022 were selected.All patients were treated with anti-infection,expectorant and other treatments for 14 days.The differences in blood white blood cell count(WBC),neutrophil count(N),C-reactive protein(CRP)level,procalcitonin(PCT)level,sputum NE activity were compared before and 14 days after treatment.The correlation between sputum NE activity and above indexes was analyzed.Results The values of the above inflammatory indexes were different before and after treatment,and all of them were lower after treatment than be-fore treatment(P＜0.05).The difference of sputum NE activity before and after treatment was positively correlated with the differences of the levels of N,CRP and PCT before and after treatment,but not with the difference of WBC.Conclusion Spu-tum NE activity is expected to be used as an index of airway inflammation to evaluate the therapeutic effect of elderly patients with acute exacerbation of bronchiectasis,and has certain clinical application value.
