      目的 探讨血栓弹力图与常规凝血检验联合应用在慢性肾脏病检查中的价值.方法 选择2020 年8 月—2022年12 月于安徽省宣城市人民医院住院治疗的80 例老年慢性肾脏病患者的临床资料进行回顾性分析.根据肾小球滤过率(Glomerular Filtration Rate,GFR)对入选者进行分组:1～2 期组(20 例)、3 期组(20 例)、4 期组(20 例)、5 期组(20例).检测并比较4 组常规凝血指标值和血栓弹力图各项指标值.结果 1～2 期组、3 期组、4 期组、5 期组组间基础资料统计值差异均无统计学意义(P＞0.05);4 组间TT、APTT、PT、INR、Fib值虽有差异,但均无统计学意义(P＞0.05);4 组组间CI、MA、K、R指标值差异明显(P＜0.05);慢性肾脏病患者eGFR水平和R、K、MA间有正相关关系(P＜0.05).结论 与常规凝血检验比较,血栓弹力图在慢性肾病患者凝血状态监测中的应用具有更高的准确性,有望为临床制定治疗方案提供参考.

      Objective To explore the value of the combined application of thrombelastogram and routine blood coagula-tion test in the examination of chronic kidney disease.Methods A retrospective analysis was conducted on the clinical data of 80 elderly patients with chronic kidney disease who were hospitalized in People's Hospital of Xuancheng City from August 2020 to December 2022.According to the glomerular filtration rate(GFR),they were divided into stage 1-2 group(20 cases),stage 3 group(20 ones),stage4 group(20 ones)and stage5 group(20 ones).The values of routine coagulation indexes and thromboelastogram indexes were detected and compared among the 4 groups.Results There was no statistically significant difference in the statistical values of the basic data among the 4 groups(P＞0.05).There were differences in the values of TT,APTT,PT,INR and Fig among the 4 groups,but there was no statistical significance(P＞0.05).There were significant differences in the values of CI,MA,K and R among the 4 groups(P＜0.05).The level of eGFR was positively correlated with R,K and MA in patients with chronic kidney disease(P＜0.05).Conclusion Compared with routine coagulation test,the application of thrombelastogram in monitoring coagulation status of patients with chronic kidney disease has higher accura-cy,which is expected to provide a reference for clinical treatment.
