      目的 分析老年消化性溃疡出血(peptic ulcer bleeding,PUB)患者接受艾司奥美拉唑镁肠溶片四联方案治疗的效果.方法 对2019 年1 月—2023 年8 月青岛市胶州中心医院收治的180 例老年PUB患者的资料进行回顾性分析,根据治疗方法不同分为观察组(n =92)及对照组(n =88).对照组予以雷尼替丁四联方案,观察组予以艾司奥美拉唑镁肠溶片四联方案,疗程均为2 周.观察并比较2 组疗效、Hp清除率、症状改善情况、胃肠激素、炎症因子、不良反应、药物经济学.结果 治疗2 周后,观察组总有效率及Hp清除率均高于对照组(P＜0.05);观察组黑便及呕血频率及GAS、TNF-α、IL-17、IL-6 水平均低于对照组(P＜0.05),SS水平高于对照组(P＜0.05);2 组不良反应发生率差异无统计学意义(P＞0.05);观察组治疗有效率的成本-效果(C/E值)低于对照组(P＜0.05).结论 艾司奥美拉唑镁肠溶片四联方案治疗老年PUB患者,效果确切,可促进Hp清除,改善患者症状及胃肠激素水平,缓解炎症反应,药物经济学价值较高,具有一定的临床应用价值.

      Objective To explore the curative effects of a quadruple regimen of esomeprazole magnesium enteric-coated tablet in elderly patients with peptic ulcer bleeding(PUB).Methods The data of 180 elderly PUB patients admitted to Jiaozhou Central Hospital of Qingdao City from January 2019 to August 2023 were retrospectively analyzed.They were divided into observation group(n =92)and control group(n =88)according to different treatment methods.The control group re-ceived a quadruple regimen of ranitidine,while the observation group received a quadruple regimen of esomeprazole magnesium enteric-coated tablets.All were treated for 2 weeks.The efficacy,Hp clearance,symptom improvement,gastrointestinal hor-mones,inflammatory factors,adverse reactions and pharmacoeconomics of the two groups were observed and compared.Results After 2 weeks of treatment,the total effective rate and Hp clearance rate of the observation group were higher than those of the control group(P＜0.05);the frequency of black stool and hematemesis and the levels of GAS,TNF-α,IL-17 and IL-6 of the observation group were lower than those of the control group(P＜0.05),and the SS level was higher than that of the control group(P＜0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P＞0.05).The cost-effectiveness(C/E value)of treatment effectiveness of the observation group was lower than that of the control group(P＜0.05).Conclusion The quadruple regimen of esomeprazole magnesium enteric-coated tablets has a defi-nite effect on the treatment of elderly PUB patients.It can promote Hp clearance rate,relieve symptoms and inflammatory re-sponse,regulate gastrointestinal hormones and has high pharmacoeconomics and certain clinical application value.
