      目的 了解老年糖尿病患者视网膜病变认知情况、影响因素及眼科健康检查意愿,为提高社区医院相关工作质量、改善糖尿病患者眼健康水平提供科学依据.方法 选择上海市黄浦区五里桥社区进行"1+1+1"家庭医生签约的老年糖尿病在册管理患者,采用自编问卷,于2021年11月-2021年12月由经过统一培训的社区医师面对面进行调查.结果 308例研究对象中,232(75.32％)例的糖尿病视网膜病变知识得分为0分,7(2.27％)例得满分(10分);二元Logistic多因素回归分析结果显示患有其他慢性疾病和有眼部疾病史的研究对象得分较高(P＜0.05);在确诊糖尿病后做过视力检查、屈光检查和眼底拍片的研究对象分别占比17.86％、11.69％和12.34％,而96.10％的研究对象表示愿意参加眼底健康筛查,且58.77％研究对象期望社区医院可电话通知其来院检查及领取报告.结论 老年糖尿病患者视网膜病变相关知识匮乏,但眼底健康筛查参与意愿较高,在该人群中开展健康教育、普及相关知识、提供适宜的眼科健康服务势在必行.

英文摘要:

      Objective To investigate the cognition of diabetic retinopathy(DR),influencing factors and willingness of ophthalmic health examination in elderly patients with diabetes,and provide scientific basis for improving the work quality of community hospitals and the ophthalmic health of diabetes patients.Methods Elderly diabetes patients with the"1+1+1"family doctor contract in Wuliqiao Community of Huangpu District were selected.A self-designed questionnaire was used to conduct face-to-face survey by community physicians who had undergone unified training from November 2021 to December 2021.Results Among the 308 subjects,232(75.32％)ones got 0 points for DR knowledge,and 7(2.27％)ones got full score(10 points).The results of binary logistic multivariate regression analysis showed that subjects with other chronic diseases and a history of ocular disease had higher scores(P＜0.05).After the diagnosis of diabetes,17.86％,11.69％and 12.34％of the subjects had undergone visual examination,refractive examination and fundus photography,respectively.96.10％of the subjects expressed that they were willing to participate in fundus health screening,and 58.77％of the subjects expected that community hospitals could call them to come to the hospital for examination and receive reports.Conclusion Elderly diabetic patients lack knowledge of DR,but they are willing to participate in fundus health screening.It is imperative to carry out health education,popularize relevant knowledge,and provide appropriate ophthalmic health services among this population.
