自制血透导管保护套在老年血透患者中的临床应用

      目的 观察自制血透导管保护套在老年血透患者中的应用效果,为老年血透患者的护理提供方法.方法 选取2022年1月-2022年12月于上海中医药大学附属曙光医院肾病科收治的老年血透导管置管患者70例,按照随机数字表法分为试验组(n=35)和对照组(n=35).试验组患者使用自制血透导管保护套进行导管保护及固定,对照组患者使用纱布、橡皮胶、纱带进行导管保护及固定.观察并比较2组导管延长管移位率、导管污染率、皮肤并发症发生率、穿刺部位出血率、患者舒适度[采用视觉模拟评分法(VAS)评估]、护理操作时间.结果 试验组导管延长管移位率低于对照组(0 vs 85.71％,P＜0.05);试验组导管污染率低于对照组(5.71％vs42.86％,P＜0.05);试验组皮肤并发症发生率低于对照组(0 vs 51.43％,P＜0.05);试验组穿刺部位出血率低于对照组(2.86％vs 25.71％,P＜0.05);试验组患者舒适度评分低于对照组[(3.97±1.22)分vs(8.64±0.92)分,P＜0.05];试验组护理操作时间短于对照组[(10.25±3.39)min vs(33.46±10.67)min,P＜0.05].结论 血透导管保护套有助于降低老年血透患者导管延长管移位率、导管污染率、皮肤并发症发生率、穿刺部位出血率,提高患者的舒适度,缩短护理操作时间,可为老年血透患者护理的一种选择方法.

      Objective To observe the application effects of self-made hemodialysis catheter protective sleeve in elderly hemodialysis patients,and provide a method for the nursing of elderly hemodialysis patients.Methods 70 elderly patients with hemodialysis catheterization admitted to Department of Nephrology of Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from January 2022 to December 2022 were selected and divided into experimental group(n=35)and control group(n=35)according to random number table method.The experimental group used self-made hemodialysis catheter protective sleeve for catheter protection and fixation,while the control group used gauze,rubber glue,and gauze tape for catheter protection and fixation.The incidence of catheter extension tube displacement,catheter contamination,skin com-plications,puncture site bleeding,as well as patient comfort[assessment using Visual Analog Scale(VAS)]and nursing oper-ation time were observed and compared between the two groups.Results The incidence of catheter extension tube displace-ment in the experimental group was lower than that in the control group(0 vs 85.71％,P＜0.05),the incidence of catheter contamination was lower than that in the control group(5.71％vs 42.86％,P＜0.05),the incidence of skin complications in the experimental group was lower than that in the control group(0 vs 51.43％,P＜0.05),and the incidence of puncture site bleeding in the experimental group was lower than that in the control group(2.86％vs 25.71％,P＜0.05).The comfort score of patients in the experimental group was lower than that in the control group[(3.97±1.22)points vs(8.64±0.92)points,P＜0.05]and the nursing operation time in the experimental group was shorter than that in the control group[(10.25±3.39)min vs(33.46±10.67)min,P＜0.05].Conclusion The hemodialysis catheter protective sleeve can help reduce the incidences of the catheter extension tube displacement,catheter contamination,skin complication,puncture site bleeding in elderly hemodialysis patients.It can also improve the comfort level of patients,shorten the nursing operation time,and would be a choice of nursing methods for elderly hemodialysis patients.
