      目的 基于高分辨磁共振(MR)的三维-多回波恢复梯度回波(3D-MERGE)序列成像评估不同性别老年下肢动脉病变患者下肢动脉形态差异.方法 回顾性分析2022年10月-2024年10月于中国人民解放军联勤保障部队第九六六医院接受下肢高分辨MR 3D-MERGE序列成像检查的116例老年下肢动脉病变患者的临床资料,根据性别分为男性组(n=67)及女性组(n=49).比较2组一般资料、生化指标及下肢动脉形态特征(最小管壁厚度、最大管壁厚度、偏心指数、平均管壁面积、管腔面积、标准化管壁指数).结果 女性组血清总胆固醇、甘油三酯、低密度脂蛋白胆固醇水平均高于男性组(P＜0.05);男性组及女性组谷丙转氨酶、谷草转氨酶、高密度脂蛋白胆固醇、血尿酸、血肌酐水平、白细胞计数、血小板计数、中性粒细胞计数比较,差异均无统计学意义(P＞0.05);男性组及女性组最小管壁厚度、最大管壁厚度、偏心指数差异均无统计学意义(P＞0.05);男性组平均管壁面积、管腔面积均高于女性组(P＜0.05),而标准化管壁指数低于女性组(P＜0.05).结论 不同性别老年下肢动脉病变患者下肢动脉形态存在明显差异,可能为临床制定性别特异性诊疗策略提供依据.

      Objective To evaluate the morphological differences in lower limb artery in the elderly with lower limb ar-tery lesions and different genders by 3-dimensional-multiple-echo recalled gradient echo(3D-MERGE)sequence imaging based on high-resolution magnetic resonance(MR).Methods A retrospective analysis was performed on the clinical data of 116 elderly patients with lower limb artery lesions who underwent high-resolution MR 3D-MERGE sequence imaging examination in Hospital 966 of PLA Joint Logistic Support Force between October 2022 and October 2024.According to gender,they were divided into male group(n=67)and female group(n=49).The general data,biochemical indexes and morphological characteristics of lower limb artery[minimal wall thickness(WTmin),maximal wall thickness(WTmax),eccentricity index(EI),mean wall area(WA),lumen area(LA),normalized wall index(NWI)]were compared between the two groups.Results The levels of serum total cholesterol,triglyceride and low-density lipoprotein cholesterol in the female group were higher than those in the male group(P＜0.05).There was no significant difference in alanine aminotransferase,aspartate ami-notransferase,high-density lipoprotein cholesterol,blood uric acid,serum creatinine,white blood cell count,platelet count and neutrophil count between the two groups(P＞0.05).There was no significant difference in WTmin,WTmax and EI between the two groups(P＞0.05).WA and LA in the male group were higher than those in the female group,and NWI was lower than that in the female group(P＜0.05).Conclusion There are significant differences in morphology of lower limb artery among elderly patients with lower limb artery lesions and different gender,which may provide a basis for formulating gender-specific diagnosis and treatment strategies in clinical practice.
