      目的 探讨银杏叶胶囊联合甲钴胺治疗老年神经性耳鸣的疗效及对炎症微循环的影响.方法 选取2022年1月-2024年1月天长市中医院门诊收治的80例老年神经性耳鸣患者,随机分为常规组(n=40,予以甲钴胺治疗)与联合组(n=40,接受银杏叶胶囊联合甲钴胺治疗).对比2组疗效、中医症候积分、甲襞微循环指标、炎症指标、中枢神经递质水平及并发症.结果 治疗后,联合组总有效率高于常规组(95.00％vs 75.00％,P＜0.05);联合组甲襞微循环总积分、炎症指标水平、5-HT水平均低于常规组(P＜0.05),而GABA水平高于常规组(P＜0.05).结论 银杏叶胶囊联合甲钴胺治疗老年神经性耳鸣疗效显著,可降低炎症微循环指标和中医症候积分,调节患者中枢神经递质水平.

      Objective To explore the efficacy of ginkgo biloba capsules combined with mecobalamin in the treatment of neurogenic tinnitus and its effects on inflammation and microcirculation in elderly patients.Methods 80 elderly patients with neurogenic tinnitus treated in Outpatient Department of Traditional Chinese Medicine(TCM)Hospital of Tianchang City from January 2022 to January 2024 were enrolled and randomly divided into conventional group(n=40,treated with mecobalamin)and combined group(n=40,treated with ginkgo biloba capsules plus mecobalamin).The therapeutic effect,TCM syndrome scores,nailfold microcirculation indicators,inflammatory indicators,central neurotransmitter levels and complications of the two groups were compared.Results The total effective rate of the combined group was higher than that of the conventional group(95.00％vs75.00％,P＜0.05).After treatment,the total score of nairfold microcirculation and the levels of inflam-matory indicators and 5-HT in the combined group were significantly lower than those in the conventional group(P＜0.05),while GABA level was higher than that in the conventional group(P＜0.05).Conclusion The combination of ginkgo biloba capsule and mecobalamin has a remarkable therapeutic effect on neurogenic tinnitus in the elderly.It can reduce inflammatory and microcirculation indicators and TCM syndrome scores,and regulate the levels of central neurotransmitters.
