      目的 探讨纤维支气管镜肺泡灌洗联合高流量氧疗治疗老年重症肺炎合并急性呼吸衰竭疗效.方法 回顾性分析2023年4月—2025年2月四川省中江县人民医院收治的100例老年重症肺炎合并急性呼吸衰竭患者的完整临床资料,根据治疗方法将患者分为观察组(n=50)和对照组(n=50).对照组患者采取高流量氧疗治疗,观察组患者采取纤维支气管镜肺泡灌洗联合高流量氧疗治疗.比较2组患者疗效,症状消失时间,治疗前后C反应蛋白(CRP)、降钙素原(PCT)及细胞介素-8(IL-8)等炎症因子水平,不良反应情况.结果 治疗后,观察组总有效率(90.00％)高于对照组(72.00％),症状消失时间短于对照组,炎症因子(CRP、PCT、IL-8)水平均低于对照组,差异均有统计学意义(P＜0.05);对照组未见不良反应,观察组出现1例胸痛,差异无统计学意义(P＞0.05).结论 纤维支气管镜肺泡灌洗联合高流量氧疗治疗老年重症肺炎合并急性呼吸衰竭,疗效良好,可降低机体炎症反应,促进患者康复,治疗较为安全.

      Objective To explore the efficacy of fiberoptic bronchoscopy alveolar lavage combined with high-flow oxy-gen therapy in the treatment of elderly patients with severe pneumonia complicated with acute respiratory failure.Methods A retrospective analysis was conducted on the complete clinical data of 100 elderly patients with severe pneumonia complicated with acute respiratory failure admitted to People's Hospital of Zhongjiang County,Sichuan Province from April 2023 to Febru-ary 2025.According to different treatment methods,they were divided into observation group(n=50)and control group(n=50).The control group received high-flow oxygen therapy alone,while the observation group received fiberoptic bronchoscopy alveolar lavage combined with high-flow oxygen therapy.The therapeutic effects,symptom disappearance time,levels of in-flammatory factors[C-reactive protein(CRP),procalcitonin(PCT),and interleukin-8(IL-8)]before and after treatment,and adverse reactions were compared between the two groups.Results After treatment,the total effective rate of the observa-tion group was higher than that of the control group,symptom disappearance time was shorter than that of the control group,and the levels of inflammatory factors(CRP,PCT,IL-8)were lower than those of the control group.The differences were sta-tistically significant(P＜0.05).No adverse reactions were observed in the control group.One case of chest pain occurred in the observation group,but the difference in the incidence of adverse reaction between the two groups was not statistically signif-icant(P＞0.05).Conclusion Fiberoptic bronchoscopy alveolar lavage combined with high-flow oxygen therapy in the treat-ment of elderly patients with severe pneumonia complicated with acute respiratory failure has a good therapeutic effect.It can re-duce the systemic inflammatory response,promote the recovery of patients,and the treatment is relatively safe.
