      目的 观察基于患者视角的共情支持联合巡诊服务安宁共照模式对老年晚期食管癌患者化疗期间心境状态、积极应对及癌因性疲乏的影响.方法 选择2021年1月—2024年1月南京医科大学附属肿瘤医院收治的80例老年晚期食管癌患者,使用随机信封法将患者分为联合组(基于患者视角的共情支持联合巡诊服务安宁共照)和常规组(常规照顾),每组40例.对比2组的心境状态[采用简明心境问卷(POMS)评估]、积极应对方式[采用医学应对方式问卷(MC-MQ)评估]、癌因性疲乏[采用Piper疲乏修订量表(PFS-R)评估].结果 化疗后,联合组患者POMS量表的消极心境维度得分低于常规组(P＜0.05),积极心境维度得分高于常规组(P＜0.05);联合组MCMQ量表面对维度评分高于常规组(P＜0.05),屈服、回避维度评分均低于常规组(P＜0.05);联合组PFS-R量表评分低于常规组(P＜0.05),血红蛋白水平高于常规组(P＜0.05).结论 基于患者视角的共情支持联合巡诊服务安宁共照模式可能有效改善老年晚期食管癌患者化疗期间的心境状态,缓解癌因性疲乏,养成积极应对方式.

      Objective To explore the effects of hospice co-care model of empathy support based on patient's perspective combined mobile medical service on mood state,positive coping and cancer-related fatigue in elderly patients with advanced e-sophageal cancer during chemotherapy.Methods 80 elderly patients with advanced esophageal cancer admitted to Affiliated Cancer Hospital of Nanjing Medical University from January 2021 to January 2024 were selected.According to random enve-lope method,they were randomly divided into combined group(empathy support based on patient's perspectives+hospice co-care based on mobile medical service)and routine group(routine care),with40 cases in each group.The mood state[Profile of Mood States(POMS)],positive coping[Medical Coping Modes Questionnaire(MCMQ)]and cancer-related fatigue[Re-vised Piper Fatigue Scale(PFS-R)]were compared between the two groups.Results After chemotherapy,the score of nega-tive mood dimension in POMS in the combined group was lower than that in the routine group,and the score of positive mood dimension was higher than that in the routine group(P＜0.05).The score of facing dimension in MCMQ in the combined group was higher than that in the routine group,and the scores of yielding and avoidance dimensions were lower than those in the routine group(P＜0.05).The score of PFS-R in the combined group was lower than that in the routine group.The hemo-globin level in the combined group was higher than that in the routine group(P＜0.05).Conclusion The hospice co-care model of empathy support based on patient's perspective combined mobile medical service can effectively improve mood state,alleviate cancer-related fatigue and develop positive coping styles in elderly patients with advanced esophageal cancer during chemotherapy.
