      目的 探讨肩颈功能锻炼方案在老年口腔癌颈部淋巴结清扫患者术后的应用效果.方法 回顾性分析2021年1月—2024年1月淮安市第一人民医院收治的100例老年口腔癌颈部淋巴结清扫患者的临床资料,按不同干预方式分为观察组(n=50)和对照组(n=50).对照组采用常规护理,观察组在与对照组相同处理的基础上加用肩颈功能锻炼方案.比较2组患者肩功能评分、心理状态和并发症情况.结果 干预后,2组患者Constant-Murley肩关节(CMS)量表总分及各维度评分较同组干预前升高(P＜0.05),且观察组CMS量表总分及各维度评分均高于对照组(P＜0.05);2组患者焦虑自评量表(SAS)、抑郁自评量表(SDS)评分均低于同组干预前(P＜0.05),且观察组SAS、SDS评分均低于对照组(P＜0.05);观察组患者颈部僵硬、瘢痕挛缩发生率均低于对照组(P＜0.05).结论 针对老年口腔癌颈部淋巴结清扫术后患者实施肩颈功能锻炼方案,可有效改善患者的肩功能障碍及心理状态,减少颈部僵硬及瘢痕挛缩的发生.

      Objective To explore the application effects of a shoulder-neck functional exercise program in elderly pa-tients with oral cancer after cervical lymph node dissection.Methods The clinical data of 100 elderly patients who underwent cervical lymph node dissection for oral cancer in Huai'an First People's Hospital from January 2021 to January 2024 were retro-spectively analyzed.They were divided into observation group(n=50)and control group(n=50)based on different inter-vention methods.The control group received routine nursing care,while the observation group received a shoulder-neck func-tional exercise program in addition to the same care as the control group.Shoulder function scores,psychological status,and complication were compared between the two groups.Results After the intervention,the total score and all dimension scores of the Constant-Murley Shoulder Joint(CMS)scale in both groups were significantly higher than those in the same group be-fore the intervention,and the observation group were higher than the control group(P＜0.05).The scores of the Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS)in both groups were significantly lower than those in the same group before the intervention,and the observation group were lower than the control group(P＜0.05).The incidence of neck stiffness and scar contracture in the observation group was significantly lower than that in the control group(P＜0.05).Conclusion The application of the shoulder-neck function exercise program in elderly patients with oral cancer who have un-dergone neck lymph node dissection can effectively improve their shoulder dysfunction and psychological state,and reduce the occurrence of neck stiffness and scar contracture.
