| 目的 比较老年经直肠超声引导下前列腺穿刺活检术(TRUS-PB)患者麻醉期间应用环泊酚和丙泊酚的呼吸相关不良事件.方法 选取2023年9月—2024年10月复旦大学附属华东医院行TRUS-PB的老年日间手术患者100例,采用随机数字表法将患者分为丙泊酚组(n=50)和环泊酚组(n=50).2组麻醉诱导均静脉注射舒芬太尼0.1 μg/kg,环泊酚组和丙泊酚组分别予环泊酚0.3 mg/kg、丙泊酚1.5 mg/kg.观察患者呼吸相关不良事件、术中低氧血症、术中低血压、心动过缓、注射痛,记录麻醉诱导时长、手术时间和苏醒时间.结果 与丙泊酚组相比,环泊酚组呼吸抑制、呼吸暂停、血氧饱和度<93%等呼吸相关不良事件构成比较少;术中低氧血症和注射痛发生率较低;麻醉诱导时长、苏醒时间较长,差异有统计学意义(P<0.05).2组术中低血压发生率、心动过缓发生率和手术时间差异无统计学意义(P>0.05).结论 丙泊酚与环泊酚均可安全用于老年日间手术TRUS-PB患者,环泊酚对呼吸系统影响更小,注射痛发生率更低. |

| Objective To compare respiratory-related adverse events during anesthesia between ciprofol and propofol in elderly patients undergoing transrectal ultrasound-guided prostate biopsy(TRUS-PB).Methods A total of 100 elderly patients scheduled for day-surgery TRUS-PB in Huadong Hospital,Fudan University,between September 2023 and October 2024 were enrolled.They were randomly divided into propofol group(n=50)and ciprofol group(n=50)according to random number method.Both groups received intravenous sufentanil(0.1 μg/kg)for anesthetic induction.The ciprofol group received cipro-fol 0.3 mg/kg,while the propofol group received propofol 1.5 mg/kg.Their respiration-related adverse events,intraoperative hypoxemia,intraoperative hypotension,bradycardia and injection pain were observed,and the anesthesia induction time,surgi-cal duration and recovery time were recorded.Results Compared with the propofol group,the ciprofol group demonstrated a significantly lower incidence of respiratory-related adverse events(respiratory depression,apnea,oxygen saturation<93%);the incidences of intraoperative hypoxemia and injection pain were also lower in the ciprofol group.Both anesthesia induction time and recovery time in the ciprofol group were longer than those in the propofol group(P<0.05).There were no statistical-ly significant differences between the two groups in the incidence of intraoperative hypotension,bradycardia,and surgical dura-tion(P>0.05).Conclusion Both propofol and ciprofol can be safely used for elderly patients undergoing day surgery TRUS-PB.Ciprofol has less impact on the respiratory system and a lower incidence of injection pain. |
