      目的 探讨术前垫枕对老年压缩性骨折患者经皮穿刺椎体成形术(PVP)后骨水泥渗漏的影响.方法 2021年5月—2023年12月,收集上海中医药大学附属龙华医院骨伤科和复旦大学附属华东医院伤外科的老年压缩性骨折患者86例,根据随机数字表法随机分为常规组(n=43)与垫枕组(n=43).常规组行PVP,垫枕组与常规组相同处理的基础上增加术前腰部垫枕.比较2组的骨水泥渗漏情况、椎体高度和视觉模拟评分法(VAS)评分.结果 86例老年压缩性骨折患者年龄60～92岁,平均(76.9±9.2)岁;其中男性13例,女性73例.与常规组相比,垫枕组骨水泥渗漏较少(76.74％vs 34.88％),差异有统计学意义(P＜0.05).PVP后,垫枕组椎体高度增幅大于常规组[(3.26±2.62)mm vs(1.31±2.58)mm,P=0.001];VAS评分降幅大于常规组[(3.14±1.71)分vs(1.70±2.89)分,P=0.005].结论 术前垫枕可能一定程度减少PVP后的骨水泥渗漏,恢复椎体高度,减轻疼痛.

      Objective To investigate the effects of preoperative pillow padding on bone cement leakage after percutane-ous vertebroplasty(PVP)in elderly patients with compression fractures.Methods From May 2021 to December 2023,86 elderly patients with compression fractures treated in Department of Orthopedics and Traumatology of Longhua Hospital Affilia-ted to Shanghai University of Traditional Chinese Medicine and Department of Traumatology of Huadong Hospital,Fudan Uni-versity were enrolled.They were randomly divided into conventional group(n=43)and pillow group(n=43)using a ran-dom number table.The conventional group underwent PVP,while the pillow group received preoperative lumbar pillow pad-ding in addition to the same PVP procedure.Bone cement leakage,vertebral height,and Visual Analogue Scale(VAS)scores were compared between the two groups.Results The 86 elderly patients with compression fractures ranged in age from 60 to 92 years(mean 76.9±9.2 years),including 13 males and 73 females.Compared with the conventional group,the pillow group had significantly less cement leakage(34.88％vs 76.74％,P＜0.05).After PVP,the increase in vertebral height in the pillow group was greater than that in the conventional group[(3.26±2.62)mm vs(1.31±2.58)mm,P=0.001],and the re-duction in VAS score was greater than that in the pillow group[(3.14±1.71)points vs(1.70±2.89)points,P=0.005].Conclusion Preoperative pillow padding may to some extent reduce bone cement leakage after PVP,restore vertebral height and relieve pain.
