化气祛瘀通幽汤治疗老年胸腰椎骨折后便秘患者的临床观察

      目的 观察化气祛瘀通幽汤治疗老年胸腰椎骨折后便秘患者的临床疗效.方法 采用随机数字表法将南充市中医医院自2023年1月—2024年8月收治的80例老年胸腰椎骨折后便秘患者分为西医组和联合组,每组40例.西医组采用常规西医治疗,联合组采用常规西医联合化气祛瘀通幽汤治疗.比较2组临床疗效、中医证候积分、便秘症状、肠道菌群、不良反应以及复发率.结果 治疗后,联合组总有效率高于西医组(P＜0.05);2组中医症候评分、排便计时、排便困难程度、排便性状评分、大肠埃希菌数量均低于同组治疗前(P＜0.05),联合组上述指标值均低于西医组(P＜0.05);2组乳杆菌、双歧杆菌数量均高于同组治疗前(P＜0.05),且联合组上述指标值均高于西医组(P＜0.05);2组不良反应差异无统计学意义(P＞0.05);联合组复发率低于西医组(P＜0.05).结论 化气祛瘀通幽汤联合西医治疗老年胸腰椎骨折后便秘患者,可显著提高临床效果,改善患者中医证候,缓解便秘症状,调节肠道菌群平衡,降低复发率.

      Objective To explore the clinical effects of Huaqi Quyu Tongyou decoction in the treatment of constipation in elderly patients after thoracolumbar vertebral fracture.Methods 80 elderly patients who had constipation after thoracolum-bar vertebral fracture and were admitted to Traditional Chinese Medicine(TCM)Hospital of Nanchong City from January 2023 to August 2024 were randomly divided into western medicine group and combination group using a random number table,with 40 cases in each group.The western medicine group received conventional western medical treatment,while the combination group was treated with Huaqi Quyu Tongyou decoction on the basis of the western medicine group.The clinical efficacy,TCM syndrome scores,constipation symptoms,intestinal flora,adverse reactions,and recurrence rates were compared between the two groups.Results The total effective rate in the combination group was significantly higher than that in the western medi-cine group(P＜0.05).After the treatment,the scores of TCM syndrome,defecation timing,defecation difficulty degree,defecation trait score and the counts of Escherichia coli in both groups were all lower than those in the same group before treat-ment(P＜0.05),and the combination group was lower than the western medicine group(P＜0.05).The counts of Lactoba-cillus and Bifidobacterium in both groups were higher than those in the same group before treatment(P＜0.05),and the com-bination group was higher than the western medicine group(P＜0.05).There was no statistically significant difference in ad-verse reactions between the two groups(P＞0.05).The recurrence rate in the combination group was significantly lower than that in the western medicine group(P＜0.05).Conclusion Huaqi Quyu Tongyou decoction combined with western medicine treatment can significantly improve clinical efficacy,alleviates TCM syndromes and constipation symptoms,regulates intestinal flora balance,and reduces recurrence rates in elderly patients with constipation after thoracolumbar vertebral fracture.
