      目的 探讨通心络联合缬沙坦治疗对老年原发性高血压患者血压及血管内皮功能的影响,为治疗该病症提供方法.方法 纳入2023年6月—2024年6月成都市新都区中医医院收治的210例老年原发性高血压患者,采用随机数字表法分为联合组(n=105)和对照组(n=105).对照组采用缬沙坦治疗,联合组在与对照组相同治疗的基础上联合通心络治疗.观察并比较2组患者治疗前、治疗3个月后血压、一氧化氮(NO)、内皮素(ET)、血管紧张素(Ang)Ⅱ、血栓素A2(TXA2)水平;比较2组不良反应发生率、治疗总有效率及预后情况.结果 治疗后,联合组治疗总有效率高于对照组,收缩压、舒张压均小于对照组,NO水平高于对照组,ET、Ang Ⅱ和TXA2水平低于对照组,差异均有统计学意义(P＜0.05);2组患者不良反应差异无统计学意义(P＞0.05).联合组血压达标率高于对照组,而冠心病发病率低于对照组,差异均有统计学意义(P＜0.05).结论 通心络联合缬沙坦治疗对老年原发性高血压患者血压水平、血管内皮功能的改善作用显著,安全性好,可改善预后.

      Objective To investigate the effects of Tongxinluo combined with valsartan on blood pressure and vascular endothelial function in elderly patients with essential hypertension,and provide a reference for the treatment of this disease.Methods 210 elderly patients with essential hypertension admitted to Traditional Chinese Medicine Hospital of Xindu District,Chengdu,between June 2023 and June 2024 were included and randomly divided into combination group(n=105)and control group(n=105)using a random number table method.The control group was treated with valsartan,while the combination group was treated with Tongxinluo in addition to the same treatment as the control group.The levels of blood pressure,nitric oxide(NO),endothelin(ET),angiotensin Ⅱ(Ang Ⅱ),and thromboxane A2(TXA2)were observed and compared between the two groups before treatment and after three months of treatment.The incidence of adverse reactions,the total effective rate and prognosis were also compared between the two groups.Results After treatment,the total effective rate of the combination group was higher than that of the control group(P＜0.05).The levels of systolic and diastolic blood pressures,ET,Ang Ⅱ,and TXA2 in the combination group were lower than those in the control group,while the level of NO was higher than that in the control group(P＜0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P＞0.05).The rate of reaching the standard blood pressure in the combination group was higher than that in the control group,while the incidence of coronary heart disease was lower than that in the control group.The differences were statistically significant(P＜0.05).Conclusion Tongxinluo combined with valsartan can significantly improve blood pressure level and vascular endothelial function in elderly patients with essential hypertension.It has good safety and can improve the prognosis of these patients.
