心理弹性对老年胃癌患者疾病进展恐惧与睡眠障碍的中介效应研究

      目的 探讨心理弹性在老年胃癌患者疾病进展恐惧与睡眠障碍间的中介效应.方法 选取2022年1月—2025年2月阜阳师范大学附属阜南医院收治的106例老年胃癌患者作为研究对象,采用一般资料量表、心理弹性量表、疾病进展恐惧简化量表及匹兹堡睡眠质量指数量表对患者进行问卷调查;采用Pearson分析法探讨心理弹性、疾病进展恐惧与睡眠障碍之间的相关性,并构建结构方程模型(SEM)以检验其中介效应.结果 106例老年胃癌患者心理弹性总分为(60.45±9.76)分,疾病进展恐惧总分为(25.92±3.51)分,睡眠障碍总分为(5.72±1.39)分;心理弹性评分与疾病进展恐惧评分呈负相关(P＜0.05),与睡眠障碍评分呈负相关(P＜0.001),而疾病进展恐惧评分与睡眠障碍评分呈正相关(P＜0.05).结论 心理弹性与老年胃癌患者的疾病进展恐惧和睡眠障碍可能存在部分中介效应,可在一定程度上减轻疾病进展恐惧对睡眠障碍的不良影响.

      Objective To explore the mediating effects of psychological resilience between fear of disease progression and sleep disorders in elderly patients with gastric cancer.Methods 106 elderly patients with gastric cancer admitted to Funan Hospital Affiliated to Fuyang Normal University from January 2022 to February 2025 were selected as the research subjects.All of them were surveyed with questionnaires including the General Information Scale,Psychological Resilience Scale,Simplified Fear of Disease Progression Scale and Pittsburgh Sleep Quality Index Scale.The Pearson analysis method was used to explore the correlations among psychological resilience,fear of disease progression and sleep disorders,and a structural equation model(SEM)was constructed to test its mediating effect.Results The total score of psychological resilience of 106 elderly patients with gastric cancer was(60.45±9.76)points,the total score of fear of disease progression score was(25.92±3.51)points,and the total score of sleep disorder score was(5.72±1.39)points.The score of psychological resilience was negatively cor-related with the score of fear of disease progression(P＜0.05),the score of psychological resilience was negatively correlated with the score of sleep disorders(P＜0.001),and the score of fear of disease progression was positively correlated with the score of sleep disorders(P＜0.05).Conclusion Psychological resilience may have a partial mediating effect on the fear of disease progression and sleep disorders in elderly patients with gastric cancer,which can alleviate the adverse effect of the fear of disease progression on sleep disorders to a certain extent.
