      目的 探索我国更年期女性抑郁现状及其与认知功能的关系.方法 回顾性选取2020年第五轮中国健康与养老追踪调查(CHARLS)中1 316名更年期女性的数据资料,根据是否抑郁分为非抑郁组(n=930)和抑郁组(n=386).分析我国更年期女性抑郁影响因素,并将年龄、居住地、上网、睡眠时长、心脏病及疼痛纳入多因素回归分析,进一步明确更年期女性抑郁状态与认知功能的关系.结果 本研究最终纳入1316名更年期女性,单因素分析发现,年龄、居住地、上网、夜间睡眠时长、心脏病及疼痛是更年期抑郁的高危因素;相关性分析提示,更年期抑郁与认知功能负相关(r=-0.21,P＜0.000 1);多因素回归分析发现,夜间睡眠时长[OR:0.762(0.698～0.831)]、疼痛[OR:2.044(1.578～2.648)]及认知功能障碍[OR:0.941(0.913～0.970)]是其高危因素,并且认知功能中定向能力及记忆能力与抑郁状态显著负相关.结论 更年期女性的抑郁状态与认知功能下降密切相关,尤其是定向能力及记忆能力在两者关系中起到了重要作用.睡眠障碍和慢性疼痛也是影响更年期女性抑郁状态的重要因素.

英文摘要:

      Objective To explore the current status of depression and its relationship with cognitive function in menopa-usal women in China.Methods The data of 1 316 menopausal women from the fifth round(2020)of the China Health and Retirement Longitudinal Study(CHARLS)were retrospectively selected.They were divided into non-depression group(n=930)and depression group(n=386)based on the presence of depressive symptoms.Factors influencing depression in menopa-usal women were analyzed.Age,place of residence,internet access,sleep duration,heart disease,and pain were included in a multivariate regression analysis to further clarify the relationship between depression and cognitive function in menopausal women.Results This study ultimately included 1 316 menopausal women.Univariate analysis revealed that age,place of resi-dence,internet access,nighttime sleep duration,heart disease and pain were high-risk factors for menopausal depression.Correla-tion analysis indicated a negative correlation between menopausal depression and cognitive function(r=-0.21,P＜0.000 1).Multivariate regression analysis identified nighttime sleep duration[OR:0.762(0.698-0.831)],pain[OR:2.044(1.578-2.648)],and cognitive dysfunction[OR:0.941(0.913-0.970)]as high-risk factors.Furthermore,the orientation ability and memory ability in cognitive function were significantly negatively correlated with the depressive state.Conclusion The depres-sive state of menopausal women is closely related to the decline in cognitive function,especially orientation ability and memory ability play an important role in it.Sleep disorders and chronic pain are also important factors affecting the depressive state of menopausal women.
