| 目的 探索不对称性等速肌力训练联合脑机接口(BCI)集成功能性电刺激(FES)对老年脑卒中患者肢体功能及神经功能的影响。方法 选取2022年3月至2024年3月首都医科大学附属北京康复医院收治的老年脑卒中患者100例,按患者意愿分为单一训练组(n=50)和联合训练组(n=50)。单一训练组接受不对称性等速肌力训练,联合训练组接受不对称性等速肌力训练联合BCI-FES。分别于干预前、干预2周后、干预4周后比较2组肢体功能及日常生活活动能力、神经功能、脑电相关指标、表面肌电信号以及不良反应发生率。结果 干预2、4周后,联合训练组Fugl-Meyer运动功能量表评分、改良Barthel指数、运动关联皮层电位、α/β波比值、积分肌电值、均方根振幅大于单一训练组(P<0.05),Wolf运动功能测试时间、美国国立卫生研究院卒中量表评分、改良Rankin量表评分小于单一训练组(P<0.05)。2组不良反应发生率差异无统计学意义(P>0.05)。结论 不对称性等速肌力训练联合BCI-FES方案能一定程度增强肢体功能及日常生活活动能力,改善神经功能和脑电相关指标。 |

| Objective To explore the effects of asymmetric isokinetic strength training combined with brain-computer interface(BCI)integrated functional electrical stimulation(FES)on limb function and neurological function in elderly stroke patients.Methods A total of 100 elderly stroke patients admitted to Beijing Rehabilitation Hospital from March 2022 to March 2024 were selected and divided into single training group(n=50)and combined training group(n=50)according to patients′wishes.The single training group received asymmetric isokinetic strength training,while the combined training group received asymmetric isokinetic strength training combined with BCI-FES.Limb function,activities of daily living,neurological function,electroencephalogram-related indicators,surface electromyography signals,and incidence of adverse reactions were compared between the two groups before intervention,after 2 weeks of intervention,and after 4 weeks of intervention.Results After 2 and 4 weeks of intervention,the Fugl-Meyer assessment scale score,modified Barthel index,movement-related cortical potentials,α/β wave ratio,integrated electromyography value,and root mean square amplitude in the combined training group were greater than those in the single training group(P<0.05),while the Wolf motor function test time,national institutes of health stroke scale score,and modified Rankin scale score were lower than those in the single training group(P<0.05).There was no statistically significant difference in adverse reactions between the two groups(P>0.05).Conclusion The combination of asymmetric isokinetic strength training and BCI-FES can enhance limb function and activities of daily living,and improve neurological function and electroencephalogram-related indicators to a certain extent. |
