老年带状疱疹后神经痛患者生命意义感调查及其危险因素研究

      目的 调查老年带状疱疹后神经痛(PHN)患者生命意义感,并分析影响其生命意义感的因素.方法 回顾性分析医院2023 年1 月至2025 年4 月收治的202 例 PHN 患者临床资料,测评患者入院时生命意义感量表(MLQ)、疼痛程度[视觉模拟评分(VAS)法]、心理状态[积极心理资本量表(PPCQ)]、社会支持程度[领悟社会支持量表(PSSS)]评分,并收集患者人口学资料(年龄、性别、文化水平、居住地、婚姻状况、医疗付费方式、疾病了解程度、病程、家庭年收入).采用多元逐步回归分析法分析影响老年PHN 患者生命意义感的因素;经Pearson 相关系数分析VAS 评分、PPCQ 评分、PSSS评分与 MLQ评分的相关性.结果 202 例老年 PHN患者 MLQ总评分为(45.08±10.49)分,VAS 评分为(6.39±1.64)分,PPCQ 评分为(92.42±19.47)分,PSSS 评分为(62.25±13.29)分.相关性分析结果显示,PHN 患者生命意义感与疼痛程度呈负相关(P<0.001),与患者心理状态、社会支持程度呈正相关(P 均<0.001).多元逐步回归分析结果显示,PSSS、病程、疾病了解程度、家庭年收入、年龄、PPCQ 均为影响 PHN 患者生命意义感的独立因素(P 均<0.05).结论 老年PHN 患者生命意义感处于中等水平,PSSS、病程、疾病了解程度、家庭年收入、年龄、PPCQ 是影响患者生命意义感的独立因素,医护人员在制定针对性干预措施时,需综合考虑患者情况,提升患者生命意义感.

      Objective To investigate the sense of meaning in life(SoMiL)in elderly patients with postherpetic neuralgia(PHN)and analyze its influencing factors.Methods The clinical data of 202 elderly PHN patients admitted to hospital from January 2023 to April 2025 were retrospectively analyzed.The scores of meaning in life questionnaire(MLQ),pain perception[visual analogue scale(VAS)],psychological status[positive psychological capital questionnaire(PPCQ)]and social support level[perceived social support scale(PSSS)]of patients were collected at admission.Their demographic data(age,gender,education level,place of residence,marital status,payment method,disease awareness,disease duration,and family annual income)were also collected.Multivariate stepwise regression analysis was used to identify factors affecting the SoMiL in elderly PHN patients.Pearson's product-moment correlation coefficient was used to analyze the correlation among the scores of VAS,PPCQ,PSSS and MLQ.Results The total scores of MLQ,VAS,PPCQ and PSSS of 202 elderly PHN patients were(45.08±10.49)points,(6.39±1.64)points,(92.42±19.47)points,and(62.25±13.29)points,respectively.Correlation analysis showed that the SoMiL in these patients was negatively correlated with pain degree(P<0.001)and positively correlated with both psychological state and social support level(both P<0.001).Multivariate stepwise regression analysis revealed that PSSS score,disease duration,disease awareness,family annual income,age,and PPCQ score were all independent factors affecting the SoMiL in elderly PHN patients(all P<0.05).Conclusion The SoMiL in elderly PHN patients is at a moderate level.PSSS,disease duration,disease awareness,family annual income,age,and PPCQ are independent factors influencing the SoMiL.When developing targeted intervention measures,medical staff should comprehensively consider the conditions of patients to improve their SoMiL.
